Have you experienced any allergic reactions and/or adverse reactions to Sildenafil (Viagra), Tadalafil (Cialis), Vardenafil (Levitra) or Avanafil (Spedra)? *

Have you previously taken Sildenafil (Viagra), Tadalafil (Cialis), Vardenafil (Levitra) or Avanafil (Spedra) on multiple occasions (minimum FOUR times), with no adverse reactions? *

  • Have you been advised to avoid strenuous activity?
  • Do you feel very breathless or experience chest pain with light to moderate physical activity, such as walking briskly for 20 minutes or climbing two flights of stairs?

Have you ever suffered from any of the conditions listed below?
  • Any heart problems including angina, chest pain, heart failure, irregular heartbeats or palpitations, heart attack (myocardial infarction), left-ventricular outflow obstruction, cardiomyopathy or valvular heart disease (e.g.aortic stenosis).
  • High Blood Pressure OR Low Blood Pressure
  • History of Stroke
  • Sight loss due to poor circulation
  • Loss of vision because of damage to the optic nerve (such as as non-arteritic anterior ischaemic optic neuropathy [NAION]) or have an inherited eye disease (such as retinitis pigmentosa)
  • Galactose intolerance, Lapp Lactase deficiency or glucose-galactose malabsorption
  • Diagnosed Liver Conditions (such as cirrhosis of the liver)
  • Diagnosed Kidney Conditions (such as reduced kidney function)
  • Bleeding Issues (such as Active Stomach ulcers (peptic or gastric ulcer), haemophilia)
  • Conditions involving the blood - Sickle Cell Anaemia, Multiple Myeloma, Leukaemia, Haemophilia
  • Have you ever experienced priapism (an erection that lasts over FOUR hours)
  • Any physical condition affecting the shape of the penis (such as angulation, Peyronie’s disease or cavernosal fibrosis)
  • Any serious medical condition which may require immediate hospitalisation

Are you currently taking any of the following medication?
  • Nitrates such as Glyceryl Trinitrate, Isosorbide Mononitrate or Isosorbide Dinitrate
  • Nicorandil
  • Recreational Nitrates such as Amyl Nitrate (also known as “poppers”)
  • Recreational Drugs illegal to possess by law
  • Antibiotics - Clarithromycin, Erythromycin, Rifampicin
  • Antifungal Medication - Ketoconazole or Itraconazole
  • Anticonvulsants - Phenobarbital, Phenytoin, Carbamazepine
  • Antivirals - Atazanavir, Etravirine, Fosamprenavir, Indinavir, Nelfinavir, Ritonavir, Saquinavir
  • Antiarrhythmic Agents (for heart rhythm control) - Amiodarone, Procainamide, Quinidine
  • Cimetidine – used to treat heart burn
  • Riociguat or other Guanylate Cyclase Stimulators for lung problems
  • Theophylline
  • Warfarin
  • Alpha Blockers (for the treatment of High Blood Pressure or Benign Prostate Hyperplasia) – Alfuzosin, Tamsulosin, Doxazosin, Prazocin, Terazocin, Indoramin)
  • Any medication to treat high blood pressure
  • Any treatment for Erectile Dysfunction apart from Sildenafil, Vardenafil, Tadalafil, Avanafil
